Declaration

In the event of an emergency we would, of course, do our utmost to contact you. However, we would draw your attention to Section 33 of the school’s Terms and Conditions (a copy of which you will have received), which reads as follows:

“Emergency Medical Treatment: The Parents authorise the Principal, or another appropriate senior member of staff with the designated authority of the Principal, to consent on behalf of the Parents to the Pupil receiving emergency medical treatment including blood transfusions within the United Kingdom, general anaesthetic and operations under the National Health Service or at a private hospital where certified by an appropriately qualified person necessary for the Pupil’s welfare and if the Parents cannot be contacted in time.

I give permission for my child to self-administer the prescribed medicine during the time they are at school. I will inform the school immediately, in writing, if there is any change in dosage or frequency of the medication or if the medicine is stopped.

I understand that it may be necessary for this medicine to be administered during Educational Visits and other out of school activities, as well as on school premises.

I agree that I am responsible for collecting any unused or out of date medicines and returning them to the pharmacy for disposal, and supplying new stock to the school, if necessary.

Consent for off-site PE Activities and Fixtures

Whilst at St Mary’s your child will be asked to participate in off-site PE activities as a part of her school curriculum, and this will include a range of activities at various venues. Your child may also be asked to take part in sports fixtures.

Details about the arrangements for sports fixtures at the Senior School are communicated to pupil’s through the team sheets posted on the PE noticeboards, and it is your child’s responsibility to communicate to you the collection times.

In order for the school to ensure the health and safety of your child whilst off-site, and to avoid you completing an off-site consent form for every fixture and every lesson we ask that you complete and return this form to cover the duration of your child’s time at St Mary’s School.

Activity: All physical education, physical activities, sports fixtures and competitions at St Mary’s School (off-site) and away at other schools.

Additional PE Information

Pupils are all given this information at the start of term but for your information.

Pupils must attend and leave school in their school uniform, then change into their PE kit at the following times:
• Before School / registration / start of lesson 1 if they have PE first thing
• Breaktime if they have PE P2 or 3
• Lunchtime if they have PE P4 or 5.

Please note - they may only go home in their PE kit if they have had a fixture that day.

Clubs will commence week beginning Monday 8 September and a list of these can be found on the PE noticeboard and on their school teams.

When we return to fixtures these will be posted on the school noticeboard outside the hall and it is the pupils’ responsibility to let their parents know if they have been selected and the pick up times etc. (Some schools are happy to have visitors onsite for watching fixtures, others are not, we will let your child know).

Your child will not be allowed to wear any valuables which includes smart watches and earrings (even if taped) for Physical Education lessons. She will need to leave those locked in her locker.

If your child is ill or injured, please provide her with a note (we ask that this is a physical one as we cannot guarantee that we will see emails in time) so that we are aware. We do have a policy where pupils must still change into their PE kit for the lesson so they can still take an active coaching/umpiring role etc and we do find this makes them still feel part of the lesson. This also helps if it is raining etc as it means their uniform will not get wet/muddy. If for any reason your child is not able to attend practical lessons e.g. she has broken her leg and cannot walk over to the grammar fields, please do let us know as soon as possible so that we can arrange a suitable class for her to join.

Please remind your child to ensure she has all her relevant medication (e.g. inhalers/Epipen) with her for lessons, it is your child's responsibility to make sure she brings this with her to each and every lesson.
